Traveling internationally with your pet requires specific documentation and advanced planning. This questionnaire helps our USDA-accredited veterinarians (Lucy Faulkner & Christine Holmes) determine the requirements needed to complete your pet’s international health certificate.

The information you provide allows our team to:

• Review country-specific USDA/APHIS and destination requirements
• Confirm timelines for exams, vaccines, lab testing, and endorsements
• Ensure your pet meets all travel regulations before your departure date

Incomplete or inaccurate information may result in travel delays or denied entry.

Instructions for Pet Owners

• Please complete all sections of this form before your appointment
• Bring any prior medical records if they were completed at another clinic
• Notify our team immediately if your travel plans change

This form must be completed before meeting with the USDA-accredited veterinarian, as the information is required to prepare your pet’s international health certificate.

Submission of the form / consultation related to the services will generate a $50 consultation fee
